Referral Agency Agreement

Referral process:

  1. Determine need for beds, furniture, and other items.

  2. Review Agreement for Services with client.

  3. Submit referral through online form (or updated method).

Houses into Homes (HiH) process:

  1. HiH receives the referral and contacts the recipient to confirm address, needs, and availability. 

  2. HiH will provide and deliver beds, furniture, and other items to households referred by the agency, to the best of our capacity and inventory.

  3. HiH will notify the referral agent when the delivery is made.

Referral Agency responsibilities:

  1. Make referrals with the best information available.

  2. Contact HiH if the referral agent becomes aware of changes to recipient contact information or address. Changes in items needed will be determined when HiH contacts the recipient household.

  3. Share information about how to make referrals with others at your agency who might make referrals. 

  4. Keep HiH informed about the contact person at your agency. 

  5. Encourage your clients/patients/members to move their items with them if they move.

It is our honor to serve your agency and your clients. You can help us better serve you and your clients by:

  • Directing offers of furniture and other items that your agency cannot use to HiH.

  • Sharing word of our collaboration when you communicate with the public (e.g. social media).
